Preheat the oven to 375 degrees.
Heat the butter in a skillet over medium heat. Add the apples and saute until they start to soften, about 5-7 minutes.
Add the cinnamon, nutmeg, coconut sugar, and lemon juice. Continue cooking, stirring periodically to prevent burning, for about 5 minutes.
Remove from heat and set aside. Lay the pie crust on a parchment paper lined baking sheet.
Place the caramels, salt and cream in a microwave safe bowl and heat for 30 seconds at a time, stirring until melted. Pour half the caramel mixture on the pie crust and spread.
Pour the apple mixture on top, leaving about 23 inches at the edge of the crust, and drizzle with the remaining melted caramel.
Fold up the edges around the apple mixture. Brush the edges with egg wash and sprinkle with the coarse granulated sugar.
Bake for 40 minutes. Remove and let cool fully before slicing.
